CS6 Save for Web - save settings & stop saving images into 'images' folder

hi,

 

using ai cs6: when save image slices using 'save web', images automatically put folder called 'images'. how stop happening?

 

the settings options used control have been removed.

 

 

also, how stop cs6 adding slice number file name (eg 'filename_03.png').

again, settings used control in cs5 gone.

further problem:

 

in save web panel, choose: export > 'selected user slices' menu on right under color table:

 

 

in save dialog appears file name has 'html' in title?:

 

 

then when click save dialog tells me file exisists (it definetly doesn't!)..

 

 

could cs6 getting hold of old cs5 save web prefs confusing it? - ie. whats 'html' in file name - thought feature removed in cs6? ive tried deleted both cs5 , cs6 prefs nothing seems work..
