Tax Codes not working

hello all,

i working on site www.ventracing.com utah company.  when recieve orders utah should charge them ut state tax 6.85%, despite best efforts @ entering tax codes properly, tax never applied orders.  idea wrong?this picture of tax codes entered.  entered several abbriviations , capitalizations of ut , utah. in effort not miss of potential entries.  idea why these not being applied?

the issue here missing enablement of "state tax" within cart layout needed have "true" parameter included in shipping option tag.

i've since added state tax applied when customer selects corresponding state "ut". 

 

 

you can go ahead , remove ut tax code clean "destination state" dropdown , include other states making tax % equal 0.
