None of the comments...contained in structure tree and other errors

1.  when running "full check" accessibility report following creation of pdf ms word, following error every page has header , footer:

          none of comments or other types of annotations on page contained in structure tree.

i've created other pdfs in same way similar headers , footers , never had problem headers & footers.  there no comments or tracked changes or similar in ms word document when converted it.

when use "find" in tags window:

     artifacts - "finds" every header & footer

     umarked annotations, unmarked links:  finds "doc id:  " text every footer

     ummarked content:  points each row of text, line, , blank line in headers & footers (individually) of each page

     unmarked comment & ocr suspects:  nothing found

 

2.  tags "doc id:  " text in footer on some pages "inaccessible link" (it's not link , why pages???)

 

3.  tab order may inconsistent structure order - received 3 of these though marked every page "use document structure" (we have found defaults don't work, have mark every single page - , particular document 180 pages long!)

 

does have idea why i'm getting these errors?  i'm following our process has worked on other documents!

 

adobe acrobat 9 pro (9.5.1) - no updates available.

windows xp

created:  ms word 2007 using acrobat add-in; acrobat ribbon --> create adobe pdf tab --> create pdf
