Javascripts – check boxes not returning proper values

hi all, not sure if right place post... here goes....

the contact form generates , e-mail.  @ moment results form checkboxes this:

 

$1_month

$6_months

$12_months

 

 

you can see aren't working... if 1 checked, show up! know there's html , js needed.... looking on web driving me round bend.

 

any simple solutions knows appreciated.

unless want them have ability click 1, 2 or 3 of boxes, best use group of radio buttons. way, 1 can clicked , value of clicked button return you.
