Having Problems with exporting to Dreamweaver

hi,

i've got 90 page doc in indesign 6, exports reasonably html.

two issues:

the toc ignored

the hyperlinks anchors ignored.

 

i've trawled pages , i'm pretty sure i've got settings right. have missed?

any gratefully received.

i've tried using articles, doesn't work - fine epub need html.

(if im not phrasing properly, please let me know, , if in wrong forum, feel free move it.).
