I want to add image name with the extension to db after upload, Can any body help?

earlier in week created php photo/image upload form clients able upload site, worked uploading image designated directory c:/path/ there more form, , write image name extension, “joplateau.jpg” example in database after upload. wanted use hiddenfield it's not working me, may not doing right. if 1 has idea how please let me know, , in advance.
